· Patients details from psychology therapy consultations (for example notes of sessions, assessments, diagnosis, treatment details, relevant situational information)
· Notes from patient information sent to Complete Psychology Ltd via the court process for Psychological Assessments
· Court Orders
· Copies of Court Bundles and legal documentation regarding patients (including but not exclusive to: child arrangement orders, positioning statements, social worker information, police reports, medical records, health visitor record, CAFCASS records, education records, statements of evidence, evidence, HST, mobile phone records, family member statements)
· Copies of Psychological Assessments (for example STAXI-2, SASSI-4, MCMI-IV, VASQ, Parent Stress Index, DAST-10, BASC 3, WASI, PCL-5, SOCRATES, PASS, PAM, AUDIT)
· Copies of Psychological Assessments, Diagnosis and Treatments
· Patient medical insurance details and correspondence with medical insurers
· Customer details of financial transactions (for example BAC payments, invoicing)
· Therapy contracts and signed copies of T&C’s
· Employee data (for example financial, contact details, next of kin and personal identifiers)

How we get personal information and why we have it?
Most of the personal information we process is provided to us directly by you for one of the following reasons:
· You are engaged in psychological therapy from Complete Psychology Ltd
. You are engaged in law proceedings for which one of our psychologists acts as an expert witness
· You are employed at Complete Psychology Ltd
We also receive personal information indirectly, from the following sources in the following scenarios:
· Courts in England and Wales
· Solicitors
· Expert Witness Agencies
· Psychology Agencies
· Local Authorities
· Health Visitors
· Social Workers
· GP’s – Medical Records

We use the information that you have given us in order to:
· Undertake Psychological Assessments
· Perform expert witness duties for Courts
· Perform expert witness duties on behalf of Psychology Agencies and Expert Witness Agencies
· To assess, diagnose and treat a variety of mental health issues
· To support employment of staff, enabling the operation of Complete Psychology Ltd
· To make renumeration payments to employees
· To ensure Complete Psychology Ltd are able to provide the correct accountancy information to Companies House and HMRC
· Following the guidance of our professional bodies; HCPC and BPC, we will undertake marketing activities

How we store your personal information
Your information is securely stored.
1) If held electronically, all data will be password protected.
2) If physically held, it will be stored in a locked filing cabinet.
How long we will keep your data
Your written records will be stored securely for 7 years after the last interaction with an adult client, and up to the age of 25 years for a child under 16 when last seen, or 26 years for 17-18 year olds.
We will keep all legal expert witness assessments and supporting documentation for 7 years or as the court instructs us.
Your data protection rights
Under data protection law, you have rights including:
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
